Dark circles under the eyes are usually caused by genetics, thin or ageing skin, pigmentation, tiredness, allergies, sun exposure, eye rubbing or shadows from under-eye hollows. They are commonly cosmetic rather than dangerous. However, sudden one-sided discoloration, significant swelling, pain, vision changes or dark bruising after an injury needs prompt medical assessment.
Dark circles, also called periorbital hyperpigmentation, describe skin beneath the eyes that looks darker than the surrounding face. The colour may appear brown, blue, purple or grey depending on skin tone and the underlying cause. Under-eye bags are different: they are swelling or tissue prominence, although bags can cast shadows that make circles look darker.
Most dark circles come from the skin, blood vessels, allergies or facial anatomy rather than the eyeball itself. An eye examination may be useful when circles occur with persistent itching, redness, watering, swelling, eye rubbing, headaches, blurred vision or suspected eyestrain. An optometrist can check the eyes and help decide whether referral to a medical doctor, dermatologist or allergy specialist is appropriate.
Treatment depends on the cause. A dermatologist may discuss topical treatment, chemical peels, lasers or fillers for selected patients. These procedures have risks and should be performed only after an individual assessment, especially because the eyelid area is delicate.
Arrange prompt assessment if discoloration appears suddenly on one side, follows a head or eye injury, or occurs with severe swelling, pain, fever, eyelid drooping, double vision, reduced vision, a bulging eye or difficulty moving the eye. Dark bruising around both eyes after significant trauma can be different from ordinary dark circles and requires urgent medical evaluation.
